Odontogenic infections can become life-threatening. The aim of the study was to determine the effectiveness of quercitin and its combination with ethylmethylhydroxypyridine succinate in the complex treatment of odontogenic infections by assessing the microbial population of the infection site. Materials and methods: The cross-sectional study included 75 patients. Purulent exudate was collected from the infection site. The number of colony-forming units was counted using the standard plate method. Results: The microbiological examination of purulent exudate obtained from the patients revealed the general prevalence of Gram-positive cocci. On the seventh day of treatment, the total number of microorganisms in the purulent exudate of patients in group I, whose treatment included a combination of the standard protocol with quercitin, significantly decreased compared to the first day of the same group. The results of treatment of patients in group II, which included the standard protocol in combination with quercitin and 2-ethyl-6-methyl-3-hydroxypyridine succinate, demonstrate a significant decrease in the total number of bacteria in the infection focus on the fifth day of treatment compared to this indicator of the group at the beginning of the study. Conclusions: When quercitin was used as part of complex treatment, the total treatment period was reduced by 1.4 days. However, the combined use of quercitin and ethylmethylhydroxypyridine succinate against the background of standard treatment of patients with odontogenic infection contributed to a reduction in hospital stay by 2 days.

THE AIM OF THE STUDY: Compare the properties of different dental implants depending on the design, shape and surface area in the experiment. MATERIALS AND METHODS: Dental implants with similar sizes (5.5×10 mm) were selected: Vitaplant VPKS, Mega Gen AnyRidge and Alpha Dent Superior Active. The calculation of the total area of the implants was performed and the implants were immersed in a ferromagnetic substance. RESULTS AND DISCUSSION: The small number of turns and their small length of the Vitaplant implant cannot create a large surface area (this size of the implant has an area of 174.7 mm2). On the thin, slightly conical body of the MegaGen implant (North Korea), the developer placed 10 turns of thread with wide blades. Due to the data design features, this implant has the largest surface area (276.5 mm2), which is an advantage in implant integration. The same number of turns (10) and a very similar frequency bring Alpha Dent implants (Germany) closer to the implant described above, but the design implements an innovative anti-rotation system. This implant size has a total surface area of 210.5 mm2. CONCLUSIONS: The implant Vitaplant VPKS is inferior in efficiency of the geometry of the implant Mega Gen AnyRidge by 24%, and the implant Alpha Dent Superior Active in turn ahead of the efficiency of the representative of the Korean company by 8.9%. The shape of the geometry of the implant affects the effectiveness of counteracting the masticatory load more than its surface area.

Introduction: Accurate determination of the patient's chronotype is one of the problems of personalized medicine. Recent studies have shown that determining of the expression of timing genes is a valuable method that can help gain molecular insight into a patient's intrinsic circadian timing. Odontogenic cellulitis is very common pathology. Since acute inflammatory diseases are an urgent pathology, the time of surgical intervention can correspond depend on the time of the patient's hospitalization. Materials and methods: The level of mRNA expression of peripheral circadian clock genes clock and bmal1, per1, cry1 in buccal epithelial cells in patients with odontogenic purulent inflammatory diseases of maxillofacial area in the morning and evening was investigated. Results: During analyzing the results of the mRNA expression study of the genes of the negative regulatory link of the peripheral molecular clock, per1 and cry1, in patients with Cellulitis of the maxillofacial area, a significant decrease (P = 0.0003) in the mRNA expression level of the cry1 gene by 2.61 times in the evening compared to its morning mRNA expression values. Conclusion: The obtained data indicate that in patients with odontogenic purulent inflammatory diseases of the maxillofacial area with an evening chronotype, a violation of the expression profile of the per1 gene in the cells of the buccal epithelium is noted, which is manifested by an increase in its evening expression in comparison with patients with a morning chronotype.

OBJECTIVE: The aim: The aim of the study was to improve preventive measures aimed at preventing the formation of pathological scarring of the skin in patients after elective surgery by intra- and postoperative use of cryopreserved placental cells. PATIENTS AND METHODS: Materials and methods: The research was conducted in the Department of Maxillofacial Surgery on the basis of the Municipal Enterprise «Poltava Regional Clinical Hospital. M.V. Sklifosovsky¼ Poltava Regional Council. A total of 60 patients took part in the study, who were hospitalized for planned surgical interventions in the department of maxillofacial surgery. RESULTS: Results: We obtained clinical data on the processes of scar formation in patients with morning and evening chronotype on the background of local intraoperative injection of cryopreserved placenta extract into the wound. We have proved that short people with an established evening chronotype need additional preventive measures, especially on the 90th and 180th days of the postoperative period. CONCLUSION: Conclusions: From our study we can say with confidence that the drug "Cryocel" reduces epithelialization time, promotes optimal wound healing and leads to the formation of more aesthetic scars, which creates optimal both functional and cosmetic results.

OBJECTIVE: The aim: Of our study was to establish how the biological rhythm of human affects the reparative functions of the body in terms of odontogenic purulent-inflammatory diseases of the maxillofacial localization. PATIENTS AND METHODS: Materials and methods: The research was conducted on the basis of the Department of Maxillofacial Surgery on the basis of «Poltava Regional Clinical Hospital. M.V. Sklifosovsky¼. A total of 40 patients with odontogenic phlegmons of maxillofacial localization. RESULTS: Results: On the first day of the study, the indicators of the clinical condition of patients did not have a significant difference in all study groups. On the 3rd day of all studied groups, the number of points probably decreased compared to the first day of the study by 22.5%, 23.1%, 23.7%, 22.7%, respectively. On day 5, we have observed a significant difference between the previous results in all groups: 1a - 26.6%, 1b - 23.8%, 2a - 23.9%, 2b - 24.0%. CONCLUSION: Conclusions: The most effective treatment results were observed in patients of the morning chronotype who underwent surgery in the morning. Thus, the influence of the morning chronotype of the circadian rhythm on the course of reparative processes is manifested in the later stages of reparative regeneration.

OBJECTIVE: The aim: To define the level of pro- and anti-inflammatory IL-1ß, IL-10 in the oral fluid of children with chronic gastroduodenitis, depending on the level of IkBα expression. PATIENTS AND METHODS: Materials and methods: Studiing the oral fluid and scraping of the gums of 50 children 6-12 years old was carried out to determine the level of IL-1ß, IL-10 and IkBα. The children were divided into 3 groups. Group 1 - 10 children with healthy periodontium and without somatic diseases. 2nd group - 20 somatically healthy children with chronic catarrhal gingivitis. The third group (20 children) of children with gastroduodenitis and chronic catarrhal gingivitis. The determination of the mRNA expression of the IkBα geneand the level of IL-10 and IL-1ß in the oral fluid was carried out by real-time PCR. RESULTS: Results: We determined that the level of pro-inflammatory IL-1ß in the oral fluid of primary school children had different levels in accordance with the state of dental and somatic health. It was the lowest in somatically healthy children without signs of gingivitis. We revealed that the level of IL-10 has anti-inflammatory properties, and an inverse relationship with the concentration of IL-1ß in the oral fluid of examined children. Analysis of IkBα expression made it possible to reveal its suppression in children of the second and third groups compared with healthy children. CONCLUSION: Conclusions: We found changes in the levels of IkBα in the gums of children with chronic catarrhal gingivitis; attenuated IkBα expression may lead to the deregulation of NF-κB pathways in the pathogenesis of periodontitis. Decreased IkBα expression may affect cytokine production and inflammatory response associated with chronic catarrhal gingivitis and with chronic gastroduodenitis.

OBJECTIVE: The aim: Determining the influence of cluster factors on the emergence and progression of caries in first- and second-grade children is appropriate in terms of determining the most significant ones. PATIENTS AND METHODS: Materials and methods: The dental status of 73 children, residents of Uzhhorod, who study in the first grades of secondary schools, has been assessed, three groups have been singled (the control group 26 healthy children). The indices of essential micro- and macronutrients in the hair, saliva, serum and urine, as well as anxiety level have been determined and the statistical analysis has been performed. RESULTS: Results: Decreased magnesium and calcium content in hair, serum, urine and mouth fluid, iodine is absent. Children of the third group have high levels of anxiety (80%), high rates of caries (14,8) and significantly reduced levels of magnesium relative to normal. CONCLUSION: Conclusions: In the patients of the main groups, a direct correlation was found between the presence of somatic pathology and the level of anxiety. In 80% of children of the third group (CSPS≥3) the level of anxiety is high In the main group patients, a direct correlation was found between the presence of somatic pathology, the level of anxiety, and the decrease in magnesium; The intensity of caries only interacts with a high level of anxiety (1.00), that is, in children who are in constant stress, the intensity of caries increases.

OBJECTIVE: The aim: Study of the influence of local oxygen deficiency on the features of the formation of keloid scars of the head and neck. PATIENTS AND METHODS: Materials and methods: The research material was 17 incisional biopsy specimens of keloid scars, which were the highest age categories of patients from 19 to 63 years. RESULTS: Results: It was revealed that at coloring according to Mallory's technique, sections of fibrinoid swelling of collagen fibers were noted near central homogeneous hyaline masses. The lateral areas of the dermis that adjacent to keloid scar tissue, at coloring by the Hart method, in our modification, were characterized by the separation of the hyalinosis zone from the undamaged dermis by coarse bundles of elastic fibers that are colored in dark purple. The undifferentiated dysplasia of the connective tissue has caused the replacement and obstruction of single vascular components, which has complicated the local oxygen deficiency of keloid-altered tissues. CONCLUSION: Conclusions: Thus, plasmaragia and the accumulation of protein deposits in the perivascular space determine the inhibition of local hemodynamics, which explains the decrease in oxygen transport to tissues. Decreased oxygenation and increased permeability of the vascular wall causes local hemocirculatory hypoxia.

Abstract Objective: To determine the effect type I collagen gene polymorphism alpha-2 (COL1A2) (rs42524) on the formation of scar tissue that is localized in the head and neck areas. Material and Methods: Sixty patients with scars in different areas of the head and neck were examined. The patients were divided into four subgroups, according to the types of scarring: G I: 15 patients with normotrophic scars; G ІІ: 15 patients with atrophic scars; G ІІІ: 15 patients with hypertrophic scars; and G IV: 15 patients with keloid scars. The age of patients ranged from 17 to 54 years. The single-nucleotide polymorphic site of the COL1A2 (rs42524) gene was detected by a polymerase chain reaction and subsequent analysis of restriction fragment lengths. Pearson's chi-squared test with Yates's correction and Fischer's exact test were used. Results: There were no significant changes between the control and basic groups (p=0.83) at analyzing the frequencies of G and C alleles. For the G allele, the calculation of odds ratio between the basic and control groups was 0.93 at 95% confidence interval (CI) (0.50-1.75), for the C allele - OR was 1.07 at 95% CI (0.57-2.01). Conclusion: Our studies may indirectly indicate the activation of the skin's protective reaction to physiological scarring and dosed scar formation in different areas of the head and neck.

OBJECTIVE: Introduction: The issue of improving the quality of diagnosis, prevention and treatment of patients with pathological scarring of the head and neck is being actively developed throughout the world. The aim of the research is to improve diagnostics of hypertrophic scars of the head and neck through the establishment of structural features of scar-modified tissues using RGB-system and ultrasound examination. PATIENTS AND METHODS: Materials and methods: In this study, 60 patients with hypertrophic scars of the head and neck were examined. Under our supervision, there were patients in whom the type of scar was previously confirmed clinically and morphologically. RESULTS: Results: Based on the optical analysis of the obtained images, it can be concluded that hypertrophic scars are well visualized and are different from intact skin. We created a standardized table of numerical values for distribution of color components in the RGB system. Analyzing the data as for the numerical differences in terms of echogenicity of hypertrophic scars, we found that the echogenicity changes in the area of medial and lateral edges of the scar had significant differences. CONCLUSION: Conclusions: A comprehensive examination of patients with hypertrophic scarring of the head and neck, which implies the mandatory use of RGB-visualization and ultrasound examination, enables us to differentiate the type of scar tissue. The described comprehensive approach to examination of patients with scarring will provide the optimal choice of methods for treatment.
